A vehicle defect or mechanical malfunction may have possibly caused a fatal car crash in San Francisco, which killed a female passenger and injured three other occupants of a taxicab. CBS News reports that the accident occurred the morning of June 14, 2010 when a taxi went out of control on an off-ramp on Highway 280, crashed and caught fire. The driver and two others got out of the taxi, but one of the occupants, a woman, was fatally injured. Officials are attempting to determine what caused this accident.
